I believe this pitch, from the National's Max Scherzer came with two out in the 9th inning, two strikes on batter Jose Tabata when Scherzer was going for a perfect game.

The pitch grazed Tabata's elbow resulting in a hit-by-pitch and wrecked the perfect game.

Scherzer went on to retire the next batter for a no-hitter.
